Health Form & TB TEst
Each employee of Endless Mountain Learning Center, INC. is required to have a physical and TB test BEFORE they will be permitted to work with children. Please make an appointment with your physician and ask them to complete the form below. If you have already had this completed and the date is no more that 6 months old, you may use it. If the physical was longer than 6 months prior to you applying, you will need an updated form completed.
