• Click “Schedule or Manage Appointment 
• Complete all forms with your information
• Choose the document that you will use to prove your identity when you have the fingerprints done.
• Next you will search for a location. Fingerprints can be done at the Montrose Hospital. To search enter 18801 as the zip code.
• When the list appears, click on Montrose or Susquehanna
  
• You can choose to schedule an appointment or simply click “Walk In”. An appointment is not necessary. 
• Print this page and take this with your license and payment. 
• Payments can be certified check, Money Order, or credit card.
• Fingerprinting should only take a few minutes. Once you have completed this and have had your fingerprints taken, you will receive a blue paper clearance in the mail.
