Child Verification Form & Directions
A child verification form is what is used to verify your hours of experience. If your experience with children is through a church setting, camp or babysitting, you will use this form to document your hours. Please print the form below and have it completed by the person you babysat for, or your supervisor. If you worked in a licensed child care setting or school, you will not need to use these forms.
